Are your contact details up to date?

If you are registered as a patient we may need to contact you regarding an appointment or test result.

Next time you contact us ask the Receptionist to check that we have your current address and telephone number.

Even if you have been at the same address for several years it is worth checking that the details we hold are up to date.

Test results

Remember that you can obtain the results of a recent test by contacting our Results line on 01233 621626 and selecting option 3 between 9.00am and 12.00 Monday to Friday.

From Sick Note to Fit Note

Since the beginning of April 2010 we have been using the new ‘Fit Note’ which has replaced the old form of Medical Certificate.

As a result of discussion with your Doctor the new form is designed to highlight specific aspects of your work that you can continue to perform and any particular support that your employer should offer to enable you to return to work.

Did Not Attend rates

In the last six months almost 1,000 appointments with our Doctors and Nurses have gone to waste as a result of patients failing to attend a booked appointment.

This is roughly equivalent to an entire week of appointments. If you are unable to make a booked appointment please try to let us know as soon as possible.
